FINAL DEADLINE for submitting AAVA Sticker Applications will be at 12:00 noon on Friday, July 17, 2015. Forms are available at the old Accounting Office or may be downloaded from the links below. NO EXTENSIONS WILL BE GRANTED.
The following documents are required for the AAVA Sticker Application:
- OR/CR and other pertinent documents (only if they have not given these to the school last June when the Transport Registration Form (TRF) was first released.
- AAVA Sticker Application Form – ONLY ONE form per carpool group; one form also for each special and outlier sticker applicant. AAVA will handle the notarization of the forms and will charge a fee of approximately P100 per form.
- Sticker Application Information Sheet – ONLY ONE sheet per carpool group; one sheet also for each special and outlier sticker applicant.
- Photocopy of most recent Community Tax Certificate or any government-issued ID
All parents/guardians shall be informed of the status of their AAVA sticker application by the close of business on Monday, July 20. Payments for the stickers and notarization fee should be made at the AAVA office.
